How to File a Request

You can file a Right-to-Know request in four ways:

  • Fax: Download the RTK Request Form fill it out, and fax it to 412-751-6002
  • Electronic Mail: Please fill out the online email form below.
  • In Person: Visit the Municipal Building Office, 522 Rock Run Road. Hours are Monday through Friday, 8:00 A.M. to 4:30 P.M.
  • U.S. Mail: Download the RTK Request Form, fill it out, and mail it to:
    Open Records Office, Municipal Building Office, 522 Rock Run Road, Elizabeth, PA 15037

The Elizabeth Township open records officer is Joelle Whiteman who can be contacted by calling the municipal building at 412-751-2880

Always retain a copy for your file. A copy of this RTK request would be necessary if you should need to file an appeal to the PA Office of Open Records upon denial. If you do not have a copy of the actual request, your appeal will be dismissed as insufficient.

RECORDS REQUESTED: Be clear and concise. Provide as mush specific detail as possible, ideally including subject matter, time frame, and type of record or party names. RTK REQUESTS SHOULD SEEK RECORDS, NOT ASK QUESTIONS.
